Creating a Community Health Plan needs a thorough assessment of the community. One step in the identification of possible health problems is through gathering the primary data from the people of the community themselves. These primary data are gathered through conducting a house-to-house survey to the six puroks of Brgy. La Concepcion.
Surveying is one of the most important activities in the second exposure. The survey tool was asked through an interview approach. Moreover, anthropometric data from children under five years of age, and blood pressure measurement in adults more than 20 years old were obtained. Observation of home, kitchen, and toilet sanitary conditions were also done.
These primary data will direct the possible community health problems in the barangay, as these are presented to the community people for prioritization. These will also be used together with the secondary data for further evaluation and assessment of the community.
However, a lot of factors must be considered while conducting the survey, namely the following:
1. Variation of geographical location from one household to the other
2. The level of willingness and cooperation of the respondents to express their true sentiments.
3. Conflict in schedule between surveying and working hours of the residents.

Nonetheless, with the 13 days allotted for the survey, the ADZU SOM students were able to survey 338 out of 362 households of the barangay based on the 2018 data, which comprises 93.87% of the total number of households.
